Armored Telehealth’s Remote Patient Monitoring (RPM) solution utilizes bluetooth and wearable devices to collect medical and other forms of health data from individuals in one location and electronically transmit that information securely to health care providers in a different location for monitoring, assessment and recommendations.
Armored’s monitoring peripherals collect a wide range of health data from the point of care, such as vital signs, weight, blood pressure, blood sugar, blood oxygen levels, heart rate, and electrocardiograms.
This data is automatically transmitted to Armored’s monitoring center via the push of a button, where every patient enrolled in the the program is monitored against the set of parameters ordered by the primary physician’s plan of care. If a patient’s readings fall outside of the set of parameters, an alarm is triggered and the monitoring center will act on the information received, contact the patient to acquire additional information, evaluate that information, and contact that patient’s Home Health Agency or Primary Care Physician, dependent on the procedures prescribed to the monitoring center by the client. In addition to the alert and alarm capabilities, long-term trends and estimated patient outcomes can be viewed by the provider giving insight as to what modifications must be made in the treatment plan to achieve improved patient outcomes.
Armored Telehealth’s RPM solution can be rapidly deployed, resulting in a significant Return On Investment (ROI) by providing Primary Care Physicians real-time information and the opportunity to timely modify, enhance, or change a plan of care resulting in a reduction in hospital readmissions/Return To Acute (RTA) rates, improved outcomes, and a reduction in overall costs.
In addition to being a sound investment as outlined above, in Texas, via Medicaid, Remote Patient Monitoring, is fully reimbursable for qualifying patients. Please Contact us for more information.
